Savoy Life is seeking a Resident Care Coordinator to provide clinical and operations support to seniors in and around Chubbuck & Pocatello, ID. The Resident Care Coordinator will work closely with an assigned Savoy Life RN as well as a Savoy provider to provide healthcare to seniors in various settings including senior living (assisted living, independent living, memory care units) as well as individual homes with potential for other settings as well (such as adult family homes).
Requirements
- High School Diploma / GED (or higher)
- 2+ years of experience in healthcare with a focus on care coordination and healthcare administrative support
- Medical Assistant, Patient Care Technician, or equivalent certification preferred, otherwise 5+ years of healthcare experience
- Experience in geriatric care or senior living community is highly desirable
- Must possess a valid and current driver's license & be comfortable driving around to different senior home settings including senior living communities as well as individual homes and beyond
- Excellent electronic medical record (EMR) documentation skills and general comfort leveraging various technologies to support day to day work
- Ability to juggle many tasks and switch between visiting with residents and coordinating behind the scenes
