Clinical Care Manager

Clinical Care Manager will provide remote health coaching to members at risk for chronic kidney disease or have chronic kidney disease. The role will involve establishing and maintaining contact with assigned and active members, assessing their health status and care coordination needs, and collaborating with their healthcare providers.

Requirements

• RN license in good standing
• A minimum of five years of RN experience
• Experience developing care plans for individuals with chronic conditions and/or Chronic Kidney Disease is highly preferred
• Bilingual in Spanish is highly preferred
• BSN is highly preferred
• CDE or CDCES highly preferred
• Experience working 100% remote as a nurse is highly preferred
