We are hiring for IDD Qualified Professional to join our team at RHA Health Services. The role involves coordinating, monitoring, and supporting the delivery of person-centered services to individuals with intellectual and developmental disabilities. The Qualified Professional serves as a key member of the interdisciplinary team and ensures the development and implementation of individualized Person-Centered Plans (PCPs).
Requirements
- Bachelor's degree in Social Work, Human Services, Psychology, Special Education, or a related field (Master's degree preferred)
- At least 2 years of experience working with individuals with intellectual and developmental disabilities; 1 year required if Master's degree is held
- Demonstrated experience in developing, writing, and implementing Person-Centered Plans
- Working knowledge of ICF regulations, person-centered planning principles, and applicable Medicaid/HCBS rules
- Excellent communication, organizational, and documentation skills
- Valid driver's license, auto insurance, and reliable transportation
