Future Opening: Care Coordinator LPN/CNA

Requirements

• Graduate of an accredited practical or vocational nursing or CNA program and licensed to practice
• 5+ years of nursing experience (home health/hospice preferred)
• Must possess and maintain a current CPR certification
• Must have a valid driver’s license and use of an insured automobile for work or access to adequate transportation
• Available for work on weekends as required
• Participate in on-call rotation for office staff
• Must be able to read, write, speak and understand English
• An ability to bend, squat, raise your arms and lift up to 50 lbs without restrictions
