Licensed Practical Nurse (LPN)

Requirements

• Pass a fingerprint criminal background check
• Pass a child abuse and neglect check
• Pass a drug screen
• Provide an original educational document showing an LPN license in good standing in Illinois
• Provide residency for the past 2 years
• Provide current and original documents verifying identity and ability to work in the US
• Provide two verifiable employment references within the last 3 years
• Provide proof of a valid drivers license
