Telephonic Nurse Case Manager

Coordinate patient care for high-risk individuals, evaluate treatment plans, discuss claims and benefits, and manage resources. Establish relationships between clients, patients, employers, providers, and attorneys. Promote quality and cost-effective care to ensure return to optimal level of work and functioning.

Requirements

• RN License in a compact state, preferably in IL
• 3+ years of TCM experience
• Experience with Workers' Compensation or Disability Management
• Strong computer skills, Microsoft Suite
• Excellent oral and written communication skills, heavy focus on customer-service
