Requirements

• Assess resident health problems and needs, develop and implement nursing care plans
• Administer medications and treatments as prescribed by physicians
• Monitor resident progress and respond to changes in condition
• Collaborate with multidisciplinary teams to provide comprehensive care
• Educate residents and their families regarding health conditions, treatment plans, and discharge
• Maintain accurate resident records and documentation in compliance with state and federal regulations, while ensuring compliance with HIPAA regulations
• Participate in case management activities to ensure continuity of care
• Provide support in specialized areas such as PT, OT, SUD, Pain Management, etc.
