Job Description
RPM Healthcare is a fast-growing company that uses proprietary software to track patients' physiological data and provide education and support. As an RPM 365 Care Coach, you'll engage patients, track data, provide education, and encourage adherence to care plans.
Requirements
- Active New York State nursing license
- Knowledge of chronic conditions and related monitoring devices
- Proficiency with technology, computer platforms, and smartphones
- Strong verbal/written communication skills
- Comfortable with patient outreach and engagement
- Highly organized, detail-oriented, and team-focused
