Case Manager - Project REACT/RESTORE

San Francisco Community Health Center is seeking a Case Manager to provide assessments, linkage to resources, and case management services for clients in the REACT and RESTORE programs. The Case Manager will work with clients to develop care plans, coordinate services, and provide supportive counseling. The ideal candidate will have experience working with diverse populations, including BIPOC and LGBTQ+ individuals, and will have knowledge of harm reduction philosophy and trauma-informed care.

Requirements

• One year of experience providing case management, counseling, advocacy, health education, service coordination, and/or outreach work to disenfranchised communities
• Experience working with diverse populations including BIPOC, the LGBTQ+ community, refugees/immigrants, people living with HIV/AIDS, people experiencing homelessness, and multiple diagnosed populations (substance use and/or behavioral health issues)
• Knowledge of harm reduction philosophy, client centered counseling, and motivational interviewing techniques and trauma informed care
• Strong verbal and written proficiency in English
• Demonstrated excellence in attendance and reliability, interpersonal, organizational, verbal, and written communication skills
