Job Description
We are looking for a Case Manager to provide utilization reviews, resource management, discharge planning, treatment plan management, and financial management in our critical illness recovery hospital. The ideal candidate will have a current licensure in a clinical discipline, previous discharge planning experience, and strong communication skills.
Requirements
- Current licensure in a clinical discipline (Nurse or Respiratory Therapist) or Social Work license
- Previous discharge planning experience
- Availability to work a minimum of 2 shifts per month on an 'as needed' basis
