Job Description
Case Manager position available at Select Specialty Hospital in Rochelle Park, NJ. The Case Manager will be responsible for utilization reviews, discharge planning, and resource management. The position requires current licensure in a clinical discipline, previous experience in an inpatient hospital setting, and ability to work on an as-needed basis.
Requirements
- Current licensure in a clinical discipline (Nurse, Respiratory Therapist, or Social Worker)
- Previous experience in an inpatient hospital setting, dealing with critical care/acute care patients
- Adequate experience in acute medical case management and confidence to manage and direct a plan of care for chronically critically ill populations
