Job Description
We are seeking a Clinical Liaison to advocate for referred patients in the pre-admission process, foster relationships with healthcare professionals, and coordinate care plans with physicians and families. You will serve as a resource for healthcare professionals and facilitate patient transfers, addressing barriers to accessing specialized post-ICU care. Full-time position with competitive salary and uncapped monthly bonus incentives, as well as a comprehensive benefits package.
Requirements
- Registered Nurse (RN) or Respiratory Therapist (RT) License
- Ability to complete clinical assessments of medically complex patients
- Demonstrated success in patient advocacy
- Superior communication and relationship building skills
