Director of Case Management (RN/PT/OT/SLP/MSW)

We are seeking a Director of Case Management to lead our care coordination efforts and improve patient outcomes. The ideal candidate will have a background in a clinical discipline, leadership experience, and knowledge of healthcare insurance and government reimbursement. The Director will oversee clinical interventions, discharge planning, and fiscal management, as well as supervise a patient caseload.

Requirements

• Current Licensure per state guidelines in clinical or related discipline
• Working knowledge of the health care insurance industry and government reimbursement
• Minimum two (2) years of experience with demonstrated health and human services leadership
