Job Description
The Case Manager provides strong clinical and case management leadership to ensure clients receive high-quality care. This role involves conducting intensive case management, evaluating patient needs, developing treatment plans, and maintaining patient records in compliance with SRC's standards. The case manager will contribute to quality improvement programs and clinical training.
Requirements
- Perform essential duties satisfactorily
- High school diploma and AA in social service field
- Current, valid California Driver License and acceptable driving record
- CPR/First Aid certification
- Ability to read, analyze, and interpret business periodicals
