Job Description
This Case Manager position provides strong clinical and case management leadership to ensure high-quality care for clients, exceeding DHS/ODR standards. The role involves intensive case management, developing treatment plans, building patient relationships, and ensuring client compliance with SRC guidelines and COVID protocols.
Requirements
- High school diploma and AA in the social service field.
- Three years’ experience working with marginalized populations.
- Timely and accurate charting skills.
- Current, valid California Driver License with an acceptable driving record.
- Current CPR/First Aid certification.
- Full English fluency (read/write/speak).
