Case Manager

Mentor Community Services provides community-based services for individuals with intellectual and developmental disabilities. The Case Manager position involves providing case management services, conducting home visits, and coordinating care with other service providers.

Requirements

• Bachelor's degree in human services or equivalent in education and experience
• One year of related work experience and knowledge of case management
• Valid driver's license, registration, and insurance
• Current CPR/First Aid Certification
• Ability to establish working relationships with individuals served
• Demonstrated competence in verbal and written communication skills
• Ability to handle crisis situations
