Job Description
Care Navigator to support residents living with dementia and their caregivers through the CMS GUIDE Model, partnering with an interdisciplinary healthcare team to provide person-centered dementia care.
Requirements
- Bachelor's degree in social work, nursing, or a related field (master's preferred)
- At least five years of experience in care coordination or case management working with individuals living with dementia and their caregivers
- Strong communication, assessment, and problem-solving skills
