Job Description
The Segue Residential Transition Program assists individuals transition out of state psychiatric hospitals by providing support for mental health treatment, developing service plans, and connecting clients to community services.
Requirements
- Assess and monitor client needs
- Develop service plans
- Connect clients to community services and resources
- Advocate for clients as necessary
- Assist clients through program enrollment and orientation
- Coordinate client medical appointments, transportation, housing, money management, etc.
- Develop and assist with client medication management plan
- Obtain and maintain entitlements for clients
- Maintain accurate and complete documentation
- Develop and coordinate transitions plans and program discharge
