Requirements

• Doctor of Pharmacy (PharmD)
• Licensed pharmacist in California
• PGY2 Ambulatory Care Residency or completion of PGY1 Residency and at least 1 year of ambulatory practice under a Collaborative Practice Agreement (CPA)
• At least 3 years of ambulatory practice with disease state management
• Strong communication, organization, and tech skills
• Empathy, integrity, and a team-first mindset
