As an RN Healthcare Advocate, you will work with Medicare patients throughout their healthcare journey to help them achieve their health and wellness goals while addressing Social Determinants of Health (SDOH).

Requirements

• RN license in good standing
• Deep understanding of Social Determinants of Health and experience working with diverse patient populations
• Strong clinical skills paired with exceptional organizational abilities
