Case Manager

Southwest Behavioral and Health Services is seeking a dedicated and empathetic Case Manager to help fulfill their mission of delivering compassionate care to enhance lives and improve communities.

Requirements

• Intake screenings (Completing intake paperwork and referrals)
• Completing annual behavioral health assessments, including service planning and support planning
• Facilitating CFTs and ARTs
• Facilitating groups
• Identifying gaps in care for our members
• Completing SMI and SED evaluation
