Case Manager

Join the AMAZING team at Southwest Network and work collaboratively with a clinical team to engage, educate, and support adults living with a serious mental illness. As a case manager, you will coordinate care and help individuals acquire skills to become self-sufficient and manage their lives.

Requirements

• Master's or bachelor's degree in a behavioral health or related field
• Bachelor's degree in a non-related field PLUS one-year related experience
• Associate degree in any field PLUS one-year related experience
• High school diploma or equivalency certificate PLUS three years' related experience
