Community Health Worker

Provides peer support and role modeling to individuals and their families to improve life skills and health/wellness. Assists clients with accessing community resources and coordinates group support programs.

Requirements

• High School Diploma or equivalent and completion of paraprofessional CHW Training Program
• CRT-Basic Life Support (BLS) - AHA American Heart Association or ARC American Red Cross
• LIC-Driver's License - State of Michigan
• CRT-Auto Insurance
