Job Description
The Community Health Worker II plays a vital role in supporting individuals and families through proactive outreach, engagement and service coordination. This position focuses on delivering a broad range of supportive services, including life skills development, employment assistance, access to meals and groceries, mental and outpatient health service navigation, transportation support, case management and housing stability resources.
