Registered Nurse Case Manager

As a Registered Nurse Case Manager at St. Croix Hospice, you will deliver individualized, holistic support to patients in their homes, collaborating with a care team to develop a personalized plan of care and ensuring exceptional symptom management with empathy and clinical excellence.

Requirements

• Associates, Bachelors, or Masters of an accredited nursing program (RN)
• 1+ years of recent acute care experience required; hospice/home health experience preferred
• Must have reliable transportation, valid driver’s license, and proof of auto insurance
