Job Description
Join St. Croix Hospice as a Registered Nurse Case Manager to deliver individualized, holistic support to patients in their homes. As a cornerstone of patient care, you'll lead meaningful end of life conversations and ensure exceptional symptom management with empathy and clinical excellence.
Requirements
- Associates, Bachelors, or Masters of an accredited nursing program (RN)
- 1+ years of recent acute care experience required; hospice/home health experience preferred
