Job Description
The Behavioral Health Group Facilitator is responsible for facilitating recovery focused psycho-educational individual / group sessions and other therapeutic interventions with assigned patients, monitoring and evaluating the effectiveness of therapeutic interventions, and participating in daily rounds to collaborate with multidisciplinary clinical staff.
Requirements
- Monitor and evaluate the effectiveness of therapeutic interventions in a group setting
- Make necessary modifications to therapeutic programming or individual interventions according to identified unit / patient needs
- Participate in daily rounds and communicate therapeutic programming needs of the individual / or unit population
- Complete timely communication and documentation of patient related information
- Plan, organize, direct, and participate in treatment programs and activities to facilitate patients’ recovery and integration into the community
- Work collaboratively with case management to provide therapeutic support individually to patient / family or other support services
- Remain current on trends in health care and mental health specialty and recovery principles
- Maintains confidentiality of all materials handled within the Network/ Entity as well as the proper release of information
