Care Manager, Older Adult Behavioral Health (Full-Time)

The Care Manager provides comprehensive care of psychiatric patients, assessing, planning, implementing, coordinating, and evaluating health and community related services to meet patient needs. The role requires effective communication, interpersonal skills, and ability to sit, stand, and walk for extended periods. The ideal candidate has 2-5 years of clinical experience in mental health, with previous experience in discharge planning and case management models preferred.

Requirements

• Master's degree in Social Work or Counseling
• Registered Professional Nurse with a current license to practice in the state of Pennsylvania.
• Licensed Social Worker, MSW
• Licensed Professional Counselor (LPC)
