Case Manager, Acute Rehabilitation - Per Diem, Primarily Mon-Fri

The Case Manager, Acute Rehabilitation provides comprehensive care to patients admitted to the inpatient acute rehabilitation unit, including securing coverage for services and facilitating safe transitions from rehab. The Case Manager assesses, plans, implements, coordinates, and evaluates health and community-related services to meet a patient's physical, psychosocial, vocational, and health needs in a cost-effective, outcomes-oriented, and timely manner.

Requirements

• MSW, RN (BSN preferred), or Masters in a related field of physical, occupational, or speech therapy
• At least two to five years of clinical experience in healthcare, preferably in a rehabilitation hospital
• Excellent communication and problem-solving skills
