Job Description
The Home Care Coordinator performs in-home safety evaluations, coordinates assessments, monitors care plan compliance, and conducts home safety evaluations to support the safe and effective delivery of in-home services.
Requirements
- California Certified Nursing Assistant (CNA) license
- CPR and First Aid certification
- A minimum of one year's experience in working with the frail or elderly population
- Experience in home care, long-term care, or community-based services preferred
