Care Manager

We are seeking a passionate and experienced Care Manager to support clients through every phase of their treatment journey, including care coordination, aftercare planning, case management, and discharge preparation. The Care Manager will play a vital role in creating a supportive environment that promotes safety, stabilization, and long-term recovery.

Requirements

• Bachelors degree in a related field preferred (required for detox level of care); high school diploma required at minimum
• Minimum of one year of experience working in the substance use disorder field
• Knowledge of addiction, mental health, treatment resources, and 12-Step recovery
• Familiarity with discharge planning, aftercare coordination, and case management
