Requirements

• 2+ years combined of related education, experience, or certification
• Community Health Worker Certification is required for positions located or serving in specific states
• Efficient and reliable transportation, including an active driver's license
• Internet Connectivity with minimum speeds
• Good communication skills
• Good organizational skills
• Strong critical thinking and problem-solving skills
• Extensive knowledge about community and available resources
