Requirements

• Serves as a liaison between multiple service providers
• Delivers culturally competent care
• Completes applications for resources and paperwork
• Follows up and tracks referrals and outcomes
• Assists with finding and scheduling transportation
• Advocates for members/patients
• Supports clinical staff
• Administers health screenings
• Coordinates care
