The Alumni Coordinator is responsible for planning and implementing alumni outreach to encourage and strengthen relationships of facility Alumni, growing Alumni referrals, maintaining and growing the CaredFor App, and creating and maintaining an alumni committee.
Requirements
- Aligns facility alumni plan with corporate alumni strategy
- Plans, coordinates, and promotes alumni and community events
- Collects alumni data upon/prior discharge and ensures the process to alumni status is smooth
- Develops and implements a communication system utilizing phone, text, email, facility app, and any other channel available to follow-up and support program for facility alumni
- Provides Alumni social media and web support
- Measures the effectiveness of the alumni programs by creating and distributing satisfaction surveys
- Runs weekly group with all discharging clients to discuss alumni services
- Conducts daily group with all clients in their first seven days of treatment to begin building a rapport and triaging client issues
