Meets Physician Standards by providing professional medical services, serving patients, managing medical support staff, providing historical reference, maintaining professional and technical knowledge, keeping informed, ensuring quality of care, and participating in outreach events.

Requirements

• Providing professional medical services to patients
• Maintaining a patient panel in line with Center standards
• Providing coordination of care and managing patient population health
• Leading the patient-centered medical home care team
• Supervising/collaborating with advanced practitioners
• Completing forms and documentation within requested time
• Participating in an ongoing basis in a Continuing Medical Education Program
• Attending committee, provider, departmental and patient-centered medical home care team meetings
• Participating in the Center’s Continuous Quality Assurance and Performance Improvement
