Advanced Practice Provider (APRN/PA)- Primary Care- Health Park

The Advanced Practice Provider (APP) provides general and specialty medical services to individuals, families and groups, emphasizing health promotion and disease prevention as well as diagnosis and management of acute and chronic diseases.

Requirements

• Provide high quality, patient-centered, comprehensive primary care services in a collaborative outpatient setting
• Diagnose and treat acute and chronic conditions, including ordering and interpreting diagnostic tests
• Develop and implement personalized treatment plans
• Educate patients and their families on disease prevention, health maintenance, and lifestyle changes
• Collaborate with physicians, specialists, and other healthcare team members to ensure high-quality care
