The Social Worker is responsible for facilitating care through effective resource coordination to help patients achieve optimal health and access to care. They conduct complex psycho-social assessments and interventions to promote timely throughput, safe discharge, and prevent avoidable readmissions.
Requirements
- Conducts complex psycho-social assessment and intervention to promote timely throughput, safe discharge and prevent avoidable readmissions.
- Ensures care is provided at the appropriate level of care based on medical necessity and assesses patients for transition needs.
- Provides in-service education for staff and collaborates with community providers to develop educational resources for staff and patients.
- Participates in planning the social work component required in selected hospital programs and facilitates TEMPO as needed.
