Responsible for facilitating care along a continuum through resource coordination, assessing patient transition needs, and conducting complex psycho-social assessments to promote timely throughput and prevent avoidable readmissions.

Requirements

• Assesses patients for transition needs and identifies those at risk for readmission
• Conducts complex psycho-social assessments and interventions
• Integrates national standards for case management scope of services
• Completes established competencies for the position within designated introductory period
