Patient Navigator - Care Coordination - 008

We are searching for a Patient Navigator to provide support in coordination of care activities and assist patients to navigate through all the steps in obtaining the necessary and appropriate care. The navigator is responsible for helping to ensure a seamless transition from inpatient to post discharge follow-up in the outpatient clinic.

Requirements

• H.S. Diploma or GED
• 1 year Experience in a healthcare setting
• Bilingual in English and Spanish (preferred)
• Experience working within a clinic setting (preferred)
