Job Description
Case Manager for Center for Schizophrenia in Cleveland, OH, providing care and care coordination to individuals with mental health needs, working collaboratively with other team members and using care coordination skills to ensure seamless care.
Requirements
- Associates degree in social work, psychology, or mental health-related field
- 3 years case management or care coordination experience
- Valid driver's license in the State of Ohio
- Ability to work closely with other team members and form effective therapeutic relationships
- Prior experience working with SPMI adults (preferred)
