Requirements
- Graduate from an accredited medical school with a degree of Doctor of Medicine or Doctor of Osteopathy.
- Board Certified or Board Eligible in Family Medicine or Internal Medicine.
- Current, valid, unrestricted license to practice medicine in the State of Ohio.
- Unrestricted DEA Registration.
- BLS Certification.
