Job Description
MDS Nurse Manager is responsible for completing and transmitting MDS Care Plans, conducting Care Plan Meetings, and creating and updating Care Plans for Assisted Living and Memory Enhancement Members. Reports directly to the Director of Healthcare.
Requirements
- Licensed in the state of Illinois as a Registered Nurse
- Current RAC-CT Certification for MDS
- Current CPR certification
- Adequate understanding of English and ability to speak and write it well
