Community Health Worker

Good Seed’s Mission is to help youth not only live but thrive! We provide supportive and specialized care for homeless youth in California through supportive housing, job training, comprehensive services, and individual planning. The Community-Based Intensive Case Management Services Team is responsible for providing resources and supports to improve overall health, social, and economic outcomes of persons served by the program.

Requirements

• Bachelor's degree in Human Services or related field
• At least 6 months of incarceration experience
• Ability to establish rapport and build trusting relationships with clients
• Experience working with diverse clientele who may have a history of extensive legal, medical, mental health, and/or substance abuse challenges.
