Case Managers

The Case Manager provides support and counseling to older youth in a transitional living environment, helping them build independent living skills and stabilize their lives.

Requirements

• Carries a caseload of residents and provides services within the scope of program regulations and agency mission.
• Evaluates and assesses independent living skills, and supports clients in meeting individual goals.
• Participates in intake, assessment, discharge, and follow-up conferences.
