Clinical Pharmacist - Formulary Management and Drug Information

This clinical pharmacist position would work 8 hour shifts in a hybrid model with a mix of on-site at the Bell Campus in Kansas City, Kansas, and work from home. The role is responsible for supporting the Pharmacy & Therapeutics Committee and Subcommittees, facilitating onboarding and standardization of drug products, managing drug shortage assessment and mitigation, and providing drug information to providers and staff.

Requirements

• Actively participates in organizational and departmental performance improvement efforts
• Actively participates on the multidisciplinary team by consistently attending patient care rounds
• Assures timely medication order processing
• Completes patient-specific medication profile reviews
• Continually reports medication incidents and adverse drug reactions
• Participates in medication reconciliation at the time of admission, transfer, and/or discharge
• Shares pharmaceutical expertise by coordinating and providing educational presentations or in-services
• Supports pharmacy residency programs, clerkship students and interns
