Requirements

• Rounding and managing the clinical care of the population at assigned SNFs.
• Prescribing medications and treatments for residents in SNFs.
• Providing high-quality, patient-centered, comprehensive primary care services to patients in SNFs.
• Timely documentation of all patient care/encounters.
• Availability for phone/secure messaging consultations with facility nursing staff.
• Reliable transportation.
• Reliable transportation required.
