Requirements
- Rounding and managing clinical care of SNF population
- Prescribing medications and treatments for residents
- Providing high-quality, patient-centered, comprehensive primary care services
- Timely documentation of patient care/encounters
- Availability for phone/secure messaging consultations with facility nursing staff
- Reliable transportation required
