As a Waiver Case Manager, you will have the opportunity to foster trusting connections with the individuals you serve. You will play a vital role in developing comprehensive care plans, coordinating services, and connecting individuals with community resources to increase their independence and overall well-being.
Requirements
- Bachelor's degree in social work and an active social work license issued by the Minnesota Board of Social Work or bachelor's degree in special education, psychology, nursing, human services or other fields related to the education or treatment of people with developmental disabilities or related conditions.
- One year of experience in the education or treatment of people with developmental disabilities or related conditions OR a minimum of one course that specifically focuses on developmental disabilities.
- Valid driver's license, reliable vehicle for work purposes, acceptable driving record, and proof of auto insurance.
- Excellent written and verbal communication skills in English.
- Ability to pass the screening required by Federal Human Services Office of Inspector General to assure that person has not been excluded from working in the Health Care Programs which receive federal funding.
