RN Care Manager - Senior Care Options

Role Overview

The RN Care Manager will ensure timely care management for members, including transitions of care, care coordination, and navigation. The nurse care manager will possess strong clinical knowledge and critical thinking skills to facilitate a care plan that ensures quality medical care for the member.

What You Will Do

Perform telephonic member outreach, conduct assessments, develop member-specific plans of care, and implement interventions. Provide health education, coordinate transitions of care, and collaborate with interdisciplinary teams.

Why It Might Be a Fit

The ideal candidate will have strong clinical and critical thinking skills, ability to work autonomously, and proficiency in Microsoft applications. Bilingual skills in Mandarin/Cantonese are preferred.

Requirements

• Registered Nurse with current unrestricted license in state of residence
• National certification in Case Management desirable
• Bachelor's degree or relevant equivalent experience
• 5 years' relevant clinical experience
• Proficiency in Microsoft applications
