SCO Nurse Care Manager

Role Overview

The SCO Nurse Care Manager will ensure timely care management across the continuum, including transitions of care, care coordination and navigation, complex case management, population health and wellness interventions, and disease/chronic condition management. The nurse care manager will work closely with members, caregivers, and providers to meet targeted member-specific goals.

What You Will Do

Perform telephonic member outreach, conduct departmental assessments, develop member-specific plans of care, implement care manager interventions, and revise plans of care as needed. Provide targeted health education, proactive strategies for condition management, and communication with key providers and vendors.

Requirements

• Registered Nurse with current unrestricted license in state of residence
• National certification in Case Management desirable
• Bachelor's degree or relevant equivalent experience
• 5 years' relevant clinical experience
• Proficiency in a second language desirable
• Experience in home care or case management
• Skill and proficiency in Microsoft applications, technical concepts and principles; computer software applications
